A Day in the Life of a Director of Home Care Professional Services

As the Director of Home Care Professional Services, you will oversee clinical operations and nursing services provided in the home setting. This high-impact leadership role is responsible for ensuring top-tier home care service delivery, regulatory compliance, and exceptional patient outcomes. You’ll lead a team of nurses and healthcare professionals, implement clinical best practices, and drive initiatives to enhance the quality of care. Your leadership will play a pivotal role in advancing the organization's mission to deliver safe, compassionate, and effective home health services.

Responsibilities include:

  • Lead and manage clinical staff providing home-based nursing care
  • Recruit, onboard, and mentor nursing professionals, promoting a positive team culture
  • Develop, implement, and oversee clinical procedures and patient care protocols
  • Ensure all services are compliant with state and federal home health regulations
  • Collaborate with physicians, case managers, and families to coordinate comprehensive patient-centered care plans
  • Conduct ongoing case audits to monitor outcomes and update care plans as needed
  • Design and launch quality improvement initiatives to elevate clinical performance
  • Oversee compliance with accrediting bodies and prepare for inspections and audits
  • Track and analyze clinical performance indicators and resource utilization
  • Build strong relationships with hospitals, skilled nursing facilities, and local healthcare providers
  • Act as a liaison between the nursing team and organizational leadership

Requirements and Qualifications:

  • Bachelor’s degree in Nursing (BSN) or Master’s degree in Nursing or Healthcare Administration preferred
  • At least 5 years of nursing experience, including 3 years in a supervisory or leadership role within home care or a similar healthcare setting
  • Current Registered Nurse (RN) license in New York, with additional licensure in New York preferred
  • Certification in Nursing Administration or Home Health Care is a plus
  • Strong clinical knowledge and familiarity with home health regulations
  • Excellent leadership, organizational, and communication skills
  • Ability to work effectively in a fast-paced, team-oriented environment
